Mansfield Vision Center LLC is committed to providing full accessibility to patient services, covering both the physical facility and non-discriminatory care practices.
Physical Facility Access
Construction Standards: New and altered facilities meet disability and OSHA regulations; existing construction complies with OSHA requirements.
Entry Control: Access to the office is restricted by the use of locks and barriers, with common areas unlocked during business hours and monitored by front office staff.
Staff Access: A maintained list tracks all employees with key or card access. Staff must never share access privileges, and all access materials are retrieved upon termination.
Patient & Visitor Oversight: Staff monitor entry and movement throughout the facility. Visitor access requires maintenance of a visitor sign-in/out log.
Restricted Areas: Areas where patients should not have access are clearly documented by appropriate notifications.
Protected Information Spaces: Rooms containing sensitive patient or practice data are physically restricted by access location or lock.
Non-Discrimination Commitment
The facility ensures equal access regardless of race, national origin, color, religion, sex, sexual orientation, gender identity, disability (physical or mental), age, status as a parent, or genetic information.
