Bring Your Insurance Card
Please bring your insurance card to every visit. Vision insurance information often does not appear on your medical insurance card.
Know Your Plan Name
Vision care plans may operate under different names than general medical insurance. If unsure, consult your HR director at work.
Copays & Allowances
Most plans include copayments for eye exams and allowances for frames, lenses, or contacts. Some plans offer set discount percentages instead.
